What to have ready

Bring what you have. If something is missing, we will help you get it.

Photo ID for each person applying
Social Security numbers
Proof of income — pay stubs or benefit letters
Proof of address — lease, mortgage, or utility bill
Immigration or residency documents, if you have them
Any insurance cards you currently hold
Recent letters from MDHHS, opened or not
Household details — who lives with you and their ages
